REMINDER: The yellow Key Card/ Hold Harmless form that was mailed to each household about 3 weeks ago MUST be signed by all members of the household who are 18 or older. This MUST be turned in by May 8 to have your keycard(s) reactivated for the pool season. The pool opens May 10. If your completed form is not at PPM in Raleigh by May 8, your card(s) will not be activated until the form is received.
You may mail or fax to PPM or drop off at 115 Kellyridge in the box on the front porch. Cathy Bishop has a small supply of blank forms if you have misplaced yours.
Your 13-17 Teen forms may also be placed in the box. All 13-17 year olds who will be going to the pool without a parent must have this form completed before they can come alone.
